Reinforcement of transdural angiogenesis in patients with cerebral perfusion deficit
강의 내용
We investigate the reinforcement of transdural angiogenesis in patients with cerebral perfusion deficits. Cerebral blood flow insufficiency poses a significant health challenge, often leading to ischemic stroke and related complications. Transdural collaterals, stemming primarily from the extracalvarial superficial temporal artery (STA) and intracalvarial middle meningeal artery (MMA) via the external carotid artery (ECA), have shown promise in revascularization strategies. However, a comprehensive understanding of their role in patients with perfusion deficits remains elusive. Employing angiopoietic agent pretreatment and multiple burrholes under local anesthesia, we aimed to enhance transdural revascularization in acute stroke patients. After several months, revascularization patterns were evaluated using transfemoral cerebral angiography. Collaterals were categorized into intracalvarial ECA-dominant, extracalvarial ECA-dominant, or balanced groups based on origin. Combination therapy allows safe and effective revascularization strategy in patients with cerebral perfusion deficit.
