Assessment 

Description: I developed a skills assesment form that breaks down each skill level per unit/sport. This form details exactly where the student skills are and what the next level of progression for that student is.

Connection: This artifact aligns with the knowledge indicator 7A, "understands the purposes, characteristics, and limitations of different types of assessments, including standardized assessments, universal screening, curriculum-based assessment, and progress monitoring tools." This form helps assess student needs, progress, growth, and outcomes. The teacher makes decisions driven by this information about curricular and instructional effectiveness. Teachers adjust lessons to meet the needs of each student. This form makes it easier for me to adjust and modify where needed. 

Importance: Having a quick form to fill out helps Teachers assess students throughout a lesson to ensure progresssion and increase skills and knowledge of physical education content. 

Description: I developed a conference simulation form for parents to have during parent teacher conferences. This form detailed exactly where their student skills were at the time of the conference. 

Connection: Performance indicator 7J, "uses assessment results to determine student performance levels, identify learning targets,

select appropriate research-based instructional strategies, and implement instruction to enhance learning outcomes." This form helps assess student needs, progress, growth, and outcomes. The teacher makes decisions driven by this information about curricular and instructional effectiveness. Teachers adjust practices to meet the needs of each student. This form makes it easier to see students within a one page snapshot. 

Importance: Teachers assess students throughout lessons, units, and the students academic career to ensure progresssion and increase skills and knowledge of physical education content.
