Specialised care calls can help and support with chronic or long-term health conditions. Independence at Home will help with managing symptoms and daily tasks.

We will put a comprehensive person-centred support plan in place and review and update this frequently as clients needs change ensuring you are receiving the correct level of support at all times.

We recognise that people with specialist care needs often require frequent medical intervention from other health professionals such as GP’s and Nurses. Independence at Home will support you in managing your care and treatment needs offering a multi-disciplinary approach with other health professionals involved in your care.
