At predetermined intervals, MDTs meet to review student progress monitoring data and determine if a change is needed. During this meeting the team will also review fidelity data collected from implementation records and observations. If student is on track to meet their goal and fidelity data indicate strong implementation, the team should make a decision to continue with instruction without modification. If fidelity data indicates poor implementation, the team should address logistic barriers or provide additional support to the interventionist to improve fidelity. If fidelity is strong, but the student is not making adequate progress-the team should make an instructional adaptation.
Team uses this form during evaluation meeting to determine if instructional changes are needed.
