Please liaise with Medical Microbiologists or Officers or contact serology laboratory for further arrangement if needed.
Referred test shall have complete request form that includes:
Significant and relevant clinical history,
Specialists' name and signature and stamp.
Incomplete form may cause rejection from the referral lab.
Request forms are the PER-Pat 301 form or special request forms that are downloadable from IMR website or MKAK website.
Additional 14 days of LTAT may be subjected due to sample delivery process and test reporting process.
