The CHSAA Physical Examination Form is below. You may use this document as your physical and have your primary health provider fill it out. Otherwise, you may use your provider's physical as well. It is your choice. However, a physical must be in possession of the school before any athlete can participate in their sport or activity. As a reminder, please upload your physical on the ERA Athletic Form.

For fees, the ERA Finance Manager will assess fees once coaches have finalized their rosters. Please do not turn-in any athletic fees during the tryout process.
