Teacher's electronic signatures will not be accepted, initials as signatures will not be accepted, and any edits made will not be accepted. Please have a look at the FAQ page for more information. 

Print the following forms out and turned into Canvas where you professor has specified.

Before you begin labs

• Complete Lab Manual quiz (see Canvas)

• Student Information Form it only needs to be filled out one time for all courses in Child Development and Education but submit screenshot in each course that requires it. 

• Contact a school district to begin your labs. 

• If you need assistance being placed at a district complete the "Student Placement Request Form"

During Labs

• Complete the video observations by the due date listed in Canvas.

• Complete your Lab Hour Record Sheet each time you observe.

• Complete Anecdotal Records Guided Notes and Records (critical reflections) and submit in Canvas by the due date.

After Labs

• Turn in your completed Lab Hour Record Sheet (16 hours are required to pass your course) 

• End-of-Semester Student evaluation (filled out by teacher at your lab site)
