This Baseline and End Line Performance Scale is the data summary form required for funding an AAC System through the Medicaid process. It should be completed at the beginning and end of all trialed AAC systems. Measurable progress in multiple observable behaviors must be demonstrated in order for the AAC trial to be deemed successful. Data collected using the above form may allow the team to accurately complete the End Line columns.
