Addressed to DOH FICT executives, central and regional office directors, BARMM Ministry of Health, chiefs of DOH facilities and LGU hospitals with Malasakit Centers, directors of specialty hospitals, reiterates that the facilities concerned need not use the CSS form prescribed by DM No. 2021-0100. Concerned facilities should instead use the standardized CES form issued through DM No. 2020-0426. Contains summary table of clarifications related to the CES and CSS related reporting.
