By authority of the Secretary of Health, signed and issued by Atty. Christian Lel M. Saquilabon, Assistant Secretary of Health, addressed to all DOH executives, central and regional directors, chiefs of DOH hospitals, treatment and rehabilitation centers, includes a link to the online survey form to facilitate compliance on or before June 22, 2026 (Monday).

This issuance originated from DOH HFDB: Address further inquiries about this issuance to the Bureau's Facility Performance Management Division.
