By authority of the Secretary of Health, signed and issued by Atty. Christian Lel M. Saquilabon, Assistant Secretary of Health, addressed to directors of DOH Centers for Health Development, attention to their respective Health Facility Development Units (HFDU), includes a link to confirm attendance of one HFDU representative for every DOH CHD.
This issuance originated from DOH HFDB: Address further inquiries about this issuance to the Bureau's Policy, Planning and Program Development Division.

All external clients of DOH Central Office bureaus and services, Centers for Health Development, DOH hospitals and treatment and rehabilitation center shall be required to fill out the FOI Standard Request Form at bit.ly/StandardFOIForm.
For details, read DM No. 2026-0157, "Utilization of the Online DOH Freedom of Information (FOI) Standard Request Form for Walk-in and Email Clients" dated February 20, 2026.
