By authority of the Secretary of Health, signed and issued by Dr. Gloria J. Balboa, Undersecretary of Health, addressed to all DOH executives, central and regional directors, directors of specialty hospitals, chiefs of DOH hospitals, treatment and rehabilitation centers, BARMM Ministry of Health, includes as annexes to the issuance HHAP Reporting Form, and Health Facility Preparedness and Service Continuity Plan lifted from DM No. 2024-0152, in aid of dissemination of info and compliance to directives.
This issuance did not originate from DOH HFDB: Address further inquiries about this issuance to the DOH's Health and Climate Change Office. The HFDB sits at the DOH's Health and Climate Change Technical Working Group.

All external clients of DOH Central Office bureaus and services, Centers for Health Development, DOH hospitals and treatment and rehabilitation center shall be required to fill out the FOI Standard Request Form at bit.ly/StandardFOIForm.
For details, read DM No. 2026-0157, "Utilization of the Online DOH Freedom of Information (FOI) Standard Request Form for Walk-in and Email Clients" dated February 20, 2026.
