By authority of the Secretary, and as in-charge of the Office of Dr. Gloria J. Balboa, Undersecretary of Health, signed and issued by Dr. Anna Marie Celina G. Garfin, Director IV of the DOH Disease Prevention and Control Bureau (DPCB), addressed to DOH executives, central and regional directors, BARMM Ministry of Health, chiefs of hospitals, heads of attached agencies, includes as annexes to the issuance a summary of exposure classification, Hantavirus Case Investigation Form, instructions on collection, submission and coordination of specimens of suspected cases of Hantavirus, and interim Hantavirus specimen referral flow, in aid of information dissemination and compliance to guidelines.
This issuance did not originate from DOH HFDB: Address further inquiries about this issuance to the DOH DPCB, and as appropriate, the other offices under Undersecretary Balboa.

All external clients of DOH Central Office bureaus and services, Centers for Health Development, DOH hospitals and treatment and rehabilitation center shall be required to fill out the FOI Standard Request Form at bit.ly/StandardFOIForm.
For details, read DM No. 2026-0157, "Utilization of the Online DOH Freedom of Information (FOI) Standard Request Form for Walk-in and Email Clients" dated February 20, 2026.
