By authority of the Secretary of Health, signed and issued by Atty. Christian Lel M. Saquilabon, Assistant Secretary of Health, addressed to DOH executives with oversight of DOH hospitals, BARMM Ministry of Health, directors of specialty hospitals, and chiefs of DOH hospitals, includes link to an online form to facilitate the registration and attendance of the intended Chief Medical Social Workers of the DOH hospitals.
This issuance originated from DOH HFDB: Address further inquiries about this issuance to the Policy, Planning and Program Development Division of the DOH HFDB.

All external clients of DOH Central Office bureaus and services, Centers for Health Development, DOH hospitals and treatment and rehabilitation center shall be required to fill out the FOI Standard Request Form at bit.ly/StandardFOIForm.
