By authority of the Secretary of Health, signed and issued by Dr. Gloria J. Balboa, Undersecretary of Health, applies to all Women and Children Protection Units of DOH, LGU, GOCC, and private hospitals, primary care facilities, and other DOH offices involved in VAWC/GBV policies, systems, and monitoring; includes as annexes to the issuance the VAWCRS process flow, the VAWC/GBV intake form, instructions or guide on the data fields in the form, and Consent for Release of Information Form in aid of compliance to these guidelines.
The issuance did not originate from DOH HFDB: Address further inquiries about this issuance to the DOH's Disease Prevention and Control Bureau.
