By authority of the Secretary of Health, signed and issued by Atty. Christian Lel Saquilabon, Assistant Secretary of Health, addressed to DOH HFSRB and CHD directors and regulatory officers, BARMM Ministry of Health and their regulatory officers, directors and owners of custodial psychiatric care facilities, DATRCs, and MFOWS, includes a link to access the online health facility preparedness survey for timely participation of the concerned health facilities on or before May 21, 2026 (Thursday).

This issuance did not originate from DOH HFDB: Address further inquiries about this issuance to the DOH Health Facilities and Services Regulatory Bureau (https://hfsrb.doh.gov.ph), or the Regulations, Licensing and Enforcement Divisions (RLED) of the DOH CHDs.
