Deadline for Submission: February 6, 2026 (Friday)
By authority of the Secretary of Health, signed and issued by Atty. Christian Lel Saquilabon, Assistant Secretary of Health, addressed to all DOH executives and regional directors, BARMM Ministry of Health, directors of specialty hospitals, and chiefs of hospitals and medical centers, includes an online link in aid of timely accomplishment and submission of the checklist.
