By authority of the Secretary of Health, signed and issued by Atty. Christian Lel Saquilabon, Assistant Secretary of Health, addressed to all DOH executives, directors of specialty hospitals, and chiefs of DOH hospitals, includes a link to the online report form to facilitate compliance to the annual accomplishment reporting of the concerned pharmacy divisions.
This issuance originated from HFDB: Address further inquiries about this issuance to the HFDB's Policy, Planning and Program Development Division.
