Adoption of Strategic Planning Forms under the Standard Forms for Procurement in Accordance with Republic Act No. 12009 and its Implementing Rules and Regulations

By authority of the Secretary of Health, signed by Atty. Christian Lel Saquilabon, Assistant Secretary of Health, addressed to DOH executives, directors, program managers, administrative officers of all concerned DOH units, as end-user units/implementing units, includes link to electronic copies of the strategic procurement planning forms, and as annex, a copy of the Market Scoping Form for reference.

Also directs EUUs/IUs of the DOH Central Office to use market scoping form as part of documentary requirements in the submission of PPMP CY 2026. DM No. 2023-0270, on the required market study in procurement planning, is repealed.

Did not originate from the HFDB; address further inquiries about this issuance to the Procurement Service.
