Deadline for Submission: June 30, 2025 (Monday)
Addressed to all DOH UHC - Health Services Cluster executives, directors of concerned bureaus and services, and regional directors from Cagayan Valley, Bicol, Western Visayas, Central Visayas, Eastern Visayas, Zamboanga Peninsula, Northern Mindanao, Davao, SOCCSKSARGEN, and Caraga, includes link for submission of accomplished GVA Tool for PCFs, and copy of the Guidance Note; includes as annex to the issuance the roles and responsibilities on the GVA Tool for PCFs under the HSRP.
Address further inquiries about this issuance to the DOH's Bureau of International Health Cooperation and/or Health Facilities Enhancement Program - Management Offices, and HFEP Units from the concerned CHDs.
