List of Universal Health Care Integration Sites Qualified for Health Care Provider Network (HCPN) Contracting

Addressed to all DOH executives, central and regional directors, chiefs of hospitals, includes as annexes the list of UHC-IS qualified for network contracting, and the assessment criteria used; directs all DOH bureaus, services and offices to provide support to successfully demonstrate integrated, functional local health systems.
Address further inquiries about this issuance to the DOH's Bureau of Local Health Systems Development.
