Dear Colleague:
My name is Melissa Blake and I am a doctoral student at Central Michigan University. For my dissertation, I am working with Dr. Troy Hicks and we are examining the adaptations of professional development (PD), educators’ perceptions of PD, and the outcomes of effective PD.
We are inviting you to participate in a survey of enacted curriculum, which was designed by the Center for Curriculum Analysis. The survey asks about educators' experiences and perceptions with professional development and their integration of skills and content.
The following questionnaire will require approximately one (1) hour to complete and will count toward one of the six hours of asynchronous requirements for the 2020 Chippewa River Writing Project “Remote Literacy Learning” Institute.
There are no foreseeable risks associated with your voluntary participation in this survey. If at any point you feel uncomfortable answering questions, you may skip them or withdraw from the survey to stop your participation. Your survey responses will be confidential -- no personal information will be collected or reported.
An IRB application has been approved by Central Michigan University’s Institutional Review Board as a “Not Human Subjects Research” project evaluation. Should you have any questions prior to, or during the survey, you can contact Dr. Troy Hicks at hicks1tw@cmich.edu.
If you are not satisfied with the manner in which this evaluation is being conducted, you may report (anonymously if you so choose) any complaints to our Teacher Education and Professional Development Department Chairperson, Dr. Kathryn Dirkin (dirki1kh@cmich.edu or 989-774-2359).
Because we may ask you to take this survey again, if you consent to do so, in December of 2020, we will ask you to create a username that is comprised of your birthday (day and month, four digits) and numbers in your street address (up to five digits) and indication of whether you are a participant (P) or facilitator (F) so we can compare and contrast results. It will follow this format
P or F (for participant or facilitator)
Two digital day of birth, and two digit month of birth
Up to five digits of your street address

For instance, if you are a participant, and your birthday is April 1 and your street address is 9876, your ID would be: P04019876

When you go to the survey link below, click on the blue Login/Register button in the upper right of the screen.
Click on the red CLICK HERE to register link at the bottom of the Login window.
On the registration screen, select Chippewa River Writing Project from the Select State or Other affiliation drop down in the green Not Registered? box on the right side and click on Register.
You will then be provided with a brief User Information Agreement, Continue.
Enter your Name, Region (Chippewa River Writing Project), Position, District, Subject, and School (Grade level is K-12 for this survey), and Submit.
Create your username (your birthday (day and month, four digits) and numbers in your street address (up to five digits) and indication of whether you are a participant (P) or facilitator (F)), password, and email address.
Continue on to the Survey Menu page and start their survey.
