Hello, my name is (NAME) and I am the Reading Partners Site Coordinator at (SCHOOL). I am so happy your student is in Reading Partners! I have made this form so that we can communicate with each other. I will use this document to send notes home with your students Take Reading Home books and book reports. Please feel free to keep the top half and return the bottom half with your thoughts, questions, or insights. I am so happy to partner with you and your student! Also, if you ever want to come and talk with me in person, I am in Room #. Please feel free to drop by and introduce yourself.
