LETTER TEMPLATES and WAIVERS

MINORS (joiners under age 18) ARE ALLOWED TO BOOK A SLOT AND BOARD WITH AOB.

LETTER OF CONSENT

• SAFETY MEASURE: A Letter of Consent and APPROVAL from the Minor’s Guardian shall be SUBMITTED UPON REGISTRATION.

• LETTER OF CONSENT TEMPLATE: AOB’s letter template is available in the Registration Form

o   2 DIFFERENT TEMPLATES: Available for Minors BOARDING WITH and BOARDING WITHOUT a Guardian/Companion

o   Download & Fill Out the appropriate AOB template provided in the Registration Form

BOARDING WITH OR WITHOUT A GUARDIAN/COMPANION

• BOARDING SOLO/ALONE - WITHOUT A GUARDIAN or COMPANION

o   REQUIRED LETTER TEMPLATE: Guardian Letter of Consent [IF BOARDING WITHOUT A GUARDIAN]

o   BE TRUTHFUL: Please be truthful with your Registration for us to ensure your safety while onboard.

o   BE RESPONSIBLE: The AOB Team (not limited to Organizers, Admins, Drivers) WILL NOT serve as your Legal Guardians on the Event Day so we are trusting you to be responsible on D-Day.

o   AOB CONTACT: Direct Contact of AOB will be provided for your Guardian’s contact – please practice DATA PRIVACY/CONFIDENTIALITY with handling AOB contact information

• BOARDING WITH A GUARDIAN or COMPANION

o   REQUIRED LETTER TEMPLATE: Guardian Letter of Consent [IF BOARDING WITH A GUARDIAN]

o   BOOKED BY A GUARDIAN/COMPANION: If you are a Guardian/Companion of a Minor and you will be the one to book for your slots and submit the Registration Form, Letter of Consent Template is still required to be accomplished and you will be completing the template on behalf of your companion who is a minor.

JOINERS WITH SPECIAL CONCERN:

• SENIORs - PLEASE SHOW SENIOR ID IF AVAILABLE BEFORE BOARDING.

• PWDs - PLEASE SHOW PWD ID or MEDICAL CERTIFICATE IF AVAILABLE BEFORE BOARDING.

• PREGNANTs - PLEASE SHOW MEDICAL CERTIFICATE IF AVAILABLE BEFORE BOARDING.

• JOINERS WITH SPECIAL/MEDICAL CONCERNS - PLEASE SHOW MEDICAL CERTIFICATE IF AVAILABLE BEFORE BOARDING.

For safety purposes, a MEDICAL LIABILITY WAIVVER shall be SUBMITTED UPON REGISTRATION. A MEDICAL CERTIFICATE IS NOT REQUIRED BUT IS APPRECIATED IF AVAILABLE.

If you are a JOINER with SPECIAL CONCERN that will be boarding with AOB, download and fill out the MEDICAL LIABILITY WAIVER TEMPLATE as required in the Registration Form. Please be truthful upon answering as this will help us to ensure your safety while onboard.
