I accept private-pay (out-of-pocket) and provide a super bill after the evaluation has been completed for you to submit to insurance for possible reimbursement.

Unfortunately, Dr. Larsson cannot provide services to those with Apple Care/Medicare/Medicaid at this time; this includes those with those insurances wishing to pay out-of-pocket.
Dr. Larsson accepts Premera and Regence insurances.
This also includes most BC/BCBS, Lifewise, and HMA plans.
Please review the patient's insurance plan to ensure Behavioral Health/Mental Health coverage is provided through one of the in-network insurance companies.
Dr. Larsson may not be considered in-network if BH/MH coverage is provided by a third-party company (United Health, Providence, and others) even if main/Medical coverage is through an in-network company.
Even with in-network/accepted insurance, there may still be out-of-pocket costs.
The patient/family would be responsible for any deductible, copay, and/or coinsurance.
Insurance eligibility and coverage is verified by Dr. Larsson approximately 24-hours prior to the intake appointment.
As services provided (billing units), insurance coverage, deducible/copay/coinsurance amounts, and insurance adjustment amount vary greatly between patients, insurance providers, and plans, Dr. Larsson is not able to provide a total out-of-pocket cost prior to the evaluation/appointments or prior to the claims being processed by insurance.
If an out-of-pocket estimate is needed prior to the intake appointment, it is recommended the patient/family verify insurance benefits directly.
The following billing/CPT codes and units are typically involved in insurance claims for neuropsychological evaluation services.
90791, 1 unit [diagnostic interview/intake]
96136, 1 unit [neuropsychological testing/scoring, 30m]
96137, 7-15 units [neuropsychological testing/scoring, each additional 30m]
96132, 1 unit [evaluation feedback/report, 60m]
96133, 3-7 units [evaluation feedback/report, each additional 60m]
Also, academic testing is not covered by insurance and is a separate, flat-rate fee of $300.00. This would include referral concerns about a specific learning disorder (SLD), reading impairment, math impairment, writing/spelling impairment, dyslexia, dyscalculia, and dysgraphia.
Although rare, if insurance were to deny claims for provided services, the patient/family would be responsible for billed services.
The average out-of-pocket cost of a typical evaluation is $2,800, ranging from approximately $2,400 to $3,400 depending on the complexity of the evaluation.
Fee Schedule:
Diagnostic Interview (1 hour): $325.00 per hour
Testing/Scoring (4-8 hours): $200.00 per hour
Report Writing (4-7 hours): $225.00 per hour
Feedback (1 hour): $225.00 per hour
