Patient Portal

This portal is designed to provide a summary of the evaluation process for each patient.

Additional information about appointments will be provided by email throughout the evaluation.

Only the scheduled Intake Date/Time (Patient ID Number) is noted within this portal. 

No identifiable PHI is noted (name, DOB, etc.) for confidentiality purposes.

Find the patient's ID number using the scheduled INTAKE APPOINTMENT.

For example, an intake appointment scheduled for February 15th at 1:00p would be shown as "2.15.1".

Information is updated manually.
Please allow 24-48 hours to reflect changes (confirmation, completed paperwork, scheduled appointment details, etc.).
