Dr. Larsson is accepting new pediatric patients (4-17) for testing and evaluation services.
Adult referrals are not accepted at this time.
Therapy services are not offered at this time.
FIRST STEPS
Complete the New Patient Form.
Confirm the scheduled appointment with Dr. Larsson by email.
Organize/Send Patient Records. This may include previous evaluations, psychiatric records, therapy notes, medical records, IEP/504, ST, OT, etc.).
Complete Intake Paperwork by 8:00a the day before the scheduled intake appointment.
INTAKE 60-minute appointment
Patient should attend.
Parent(s)/guardian(s) should attend, if the patient is a minor.
Information about the referral concern(s), medical history, psychiatric history, psychosocial history, and educational/work history will be gathered.
Evaluation procedures will be reviewed, consent/assent will be obtained, and insurance/payment information will be verified. An estimated out-of-pocket cost will be provided. If using insurance, the patient/family can also verify coverage and potential cost using information provided on the intake paperwork.
Prior authorization from insurance will be requested, if needed, based on information gathered during the intake appointment.
TESTING 3- to 6-hour appointment(s)
iPad, computer, and paper questions, puzzles, and “games” one-on-one with Dr. Larsson.
Verbal skills, visuospatial skills, reasoning, processing speed, attention, memory and learning, social behaviors, emotional functioning, and personality.
Academic testing can be included (for students).
Complete RATING SCALES.
Parent(s)/guardian(s), self, teachers, and others.
Rating scales should be completed as soon as possible, as they are a very important piece to the comprehensive evaluation.
FEEDBACK 60-minute appointment
Parent(s)/guardian(s) should attend, if the patient is a minor.
Patients can attend, if a minor. It is recommended patients 12 and older do consider attending.
Testing results, strengths and weaknesses, and diagnoses will be discussed.
Recommendations, treatment plan suggestions, and resources will be reviewed.
Receive WRITTEN DIAGNOSTIC REPORT.
The diagnostic report will be finalized and released by HIPAA-compliant email after the feedback appointment, typically within 5-10 business days.
Resources applicable to the patient will be shared with the report.

For intake and feedback appointments, 2-hour street parking is available along Colby Ave.
Enter the main doors off of Colby, pass through the lobby on the main level, and continue to the left to find the SPA offices at the end of the hallway.
I will greet you in the waiting room at your appointment time.
Please note, if you cancel an appointment within 24-hours of the appointment time, the appointment is rescheduled because paperwork is not completed by 8:00a the day before the appointment, or in the event of a no-show, a fee of $75.00 will be applied to the account and this fee will be due before the appointment can be rescheduled.
