Depending on your type of business you may need different types of forms in your business. In this section, I'll be going over forms needed by beauty service professionals .
Some insurance companies have forms they recommend you to use in your business. Contact your insurance company or check out thier website to learn more.
Note: I am not a lawyer nor do I provide legal advise. We recommend having a lawyer look over and approve your forms to be sure they are up to legal requirements in your field and state.
Types of Forms
Policy Acknowledgement Form - This is a document that a client signs to signify they have read, understand, and comply with all policies held by the said business. They should receive a copy of the Policies and signed document.
Why? This is great to have handy, and is not typically required, that way your client has all policy outlines your business has and they cannot say they were unaware of said policies especially when it comes to disputes.
In-take Forms - These forms are filled out by the client prior to the service or consultations to clarify what services the client needs and what their concerns are. This helps the client to become involved in the treatment process. These indications will also help the service provider determine what the clients needs and desires are, problem areas, etc.
Consent forms - These documents are signed by the client prior to the service to be had in which to show that they will allow something (service) to happen. These documents also help protect the business from potential legal issues by demonstrating the client willingly agreed to said services and understood associated risks.
Photo/video Release - These documents are signed by the client prior to the service -or- prior to any photos or videos of the client are taken. This gives your and your business permission to keep the photos on the clients internal file and/or permission to use the photo/video for marketing or educational purposes.
Medical History - These forms are essential when it comes to the beauty industry. These forms must be completed by the client prior to any service or during a consultation. This allows the service provider to see if the client is suitable for the desired treatment or what they could be best suited for, this allows allows service providers to see any potential risks, discuss expectations, treatment options, and making sure you understand them and their needs.
Credit Card Authorization Form - A credit card authorization form is a document that customers (or cardholders) fill out to grant businesses the permission to charge their credit/debit card (ex. cancellation fees) and/or keep them on file.
