In-Processing Guidance for the Department of Surgery
To accelerate the in-processing for USU Walter Reed Surgery, we offer the following guidance.
Getting an USU Email address
To start the process of getting an USU Email address:
Please fill out the USU Account Request form and DD2875 digitally, these forms must be filled out digitally, you will need to use Adobe Acrobat or Adobe Reader.
Scanned forms will not be accepted.You may need to take the Cyber Awareness Training first.
If you have already taken the training before and you have a copy of your certificate, you can use that as long as it is less than a year old.On Page 1 of the USU Account Request form, fill in as much of the top section (Applicant Information) as you can.
Please make sure you have access to your "Alternate (personal) Email address", your temporary password for your USU Email account will be sent to that address.Read Pages 2 through 4 and acknowledge your agreement to the USU Computer User Agreement.
On the DD2875, just fill out Part 1 (blocks 1 to 12) and blocks 14 - 16.
You do not need to sign the document, we can apply a digital signature on your behalf. If you do decide to sign the document, please be sure not to lock it. We will return locked documents as we will not be able to process it.
Please email the completed USU Account Request Form and DD2875 along with your Cyber Awareness Training Certificate to SUR-Dept-POC@usuhs.edu for further processing.
In-Processing with USU Security
At this time, USU Security is employing Remote New Employee Check-in:
Complete the electronic USU Employee Profile Sheet digitally (do not provide Social Security Number)
Email badge application to appropriate email address:
Contractor: email form to Contractor-Onboarding@usuhs.edu and must include a copy of their offer letter.
Military: email form to SecurityDept@usuhs.edu and must include the check in sheet that MPO provides and a copy of their orders or faculty appointment letter.
Civilian: will be on-boarded by CHR and may skip this step.
Call USU Security's main line 301-295-3033 to provide your Social Security Number for clearance verification.
Getting or Renewing a Common Access Card (CAC)
Please go the ID Card Office Online to make an appointment to get your CAC.
Federal Employees:
Federal employees can get their CAC once they receive their SF-52 showing the start of government service.
New Contractors:
New contractors can apply their CAC once they get their USU Email address.
Please fill out the top portion of the USU Contractor CAC Application.
Please send completed USU Contractor CAC Application to SUR-AO@usuhs.edu for signature and submission.
Contractors will received further instructions from USU Security once they are in the system and can go get their CAC.
Existing Contractors:
Contractors with expiring CAC can renew their CAC by filling out the portion of the USU Contractor CAC Application.
Please send completed USU Contractor CAC Application to SUR-AO@usuhs.edu for signature and submission.
Contractors will received further instructions from USU Security once they are in the system and can go get their CAC.
Base Access
If you need access to the NSA-Bethesda campus, please refer to Base Access under Resources to further guidance. Please makes sure to indicate you are a new employee and will need a 30-days pass in your Base Access request.
