Campus Access

Campus Access Procedures for the Department of Surgery

If you or your guest needs campus access, please email the following items to SUR-Base-Access@usuhs.edu :

• Date of Access: (if multiple days is request, please indicate start and end date)

• Visitor’s Name: (last, first as it appears on the visitor’s driver’s license or government-issued id) 

• Company (if your guest is visiting on behalf of their organization, or n/a)

• Destination on base

• Email address of the guest needing campus access

• Please indicate if a parking pass is required, we will do our best to accommodate.

Please plan ahead and allow 48 hours advance notice.

We will blind copy all parties involved in the campus access request.

To save time at the gate, please have your guest fill out, print, sign, and bring the SECNAV 5512 form to Gate 1 when picking up their pass.
