Accomplish the NTTCHP Application for Graduation form to be included in the tentative list of candidates for graduation.
Ask your program adviser to sign the NTTCHP Application for Graduation form.
Access the Google form of the Application for Graduation to fill out the fields and upload the signed Application for Graduation form.
Wait for the acknowledgement of the Student Records Evaluator (SRE) of your submission to complete the checklist of the requirements for graduation.

Presenting in the Graduates Studetns' Colloquium (GSC) is one of the requirements for graduation for students who are working on their thesis or dissertation.

If you are eligible and would like to be included in the list of presenters to be submitted to NGOHS, kindly accomplish the Google form for NTTCHP OCS Student Requests Google form to inform OCS about this. Wait for the acknowledgement of the Student Records Evaluator (SRE) to inform you about the annuual GSC schedule and requirements.
Please prepare the following:
Abstract
Pre-recorded video presentation (20 minutes) Note: Every presentation during the colloquium will be followed by a Q&A portion for 10 minutes.
Information and photo to be used in the slide decks
Presentation release form (This will be attached in the formal invitation letter.)

Fill out the College Clearance and University Clearance with your personal details and purpose for application.
Access the Google form of the NTTCHP OCS Student Requests to upload the filled out College and University Clearances to request for processing.
The SRE will be the one to forward them to the signatories.
The College Secretary will sign the College Clearance once all the requirements of Graduation have been submitted. Please send the Checklist of Requirements for Graduation with your signature when you upload the clearance forms in the Google form for NTTCHP OCS Student Requests.
Note: The NTTCHP Library would like to have the PDF copy of the final manuscript of the capstone/thesis/dissertation before they sign the College Clearance. We can share to them the PDF copy you will upload in the Google form of the NTTCHP Requirements for Graduation.
Once the University Clearance is fully signed, the SRE will forward it to OUR.
Pay the graduation fee of Php 950.00 using any of the payment instructions from the Cash Office (Click here fo rthe infographic.)

Format and Style for the Capstone Project [Since APA format would still be followed in a capstone, please check the guidelines in the UPM Manual for Format and Style of Thesis and Dissertation 2013 for formatting.]

Wait for the Student Records Evaluator (SRE) to acknowledge the request. The SRE will then forward the certificate to the signatories.
NGOHS will only release the hardbound copies signed by the Directory once they receive this certification.
