Requirements for Shifting to CS

A. Application letter addressed to the Department Chair/ Program Coordinators of the degree program the student applied for.

For BS Biology:

The letter should be signed by the student and his/her parent/guardian, and noted by the program adviser, department chair/ program coordinator, and the College Secretary of the present college. For applicants within CS, there is no need for the signature of the College Secretary. The letter should also include the following:

• • • Justification for shifting/transfer (detailing your reasons for intending to shift or to transfer)

    • Student’s contact number(s) and email address
      
      

B. Certification of General Weighted Average (GWA) 

Aside from the units earned, additional requirements per degree program:

BS BIOLOGY

• GWA of 2.000 or better

• Successful completion of a qualifying examination

BS COMPUTER SCIENCE

• GWA of 2.000 or better

BS MATHEMATICS

• GWA of 2.000 or better

BS STATISTICS

• Grade average of 2.000 or better in her/his Mathematics and/or Statistics courses

Note for S1 & S2: All documents should be submitted in a single PDF file with the file name: Surname_Shiftee_DegreeProgramAppliedFor.pdf (e.g. DelaCruz_Shiftee_BSBIO.pdf).

Note for T1: All documents should be submitted BOTH in printed and digital copies.

Printed copies:

• • Should be sent to CS-OCS mailing address through courier from Monday to Friday (8:00 AM to 5:00 PM)
