Requirements for Shifting to CS

A. Accomplished Application for Shifting/Transfer Form (OUR Form 2021-6)

B. Application letter addressed to the College Secretary

The letter should be signed by the student and his/her parent/guardian, and noted by the academic adviser, department chair/ program coordinator, and the College Secretary of the present college. For applicants within CS, there is no need for the signature of the College Secretary. The letter should also include the following:

• • • Justification for shifting/transfer (detailing your reasons for intending to shift or to transfer)

    • Student’s contact number(s) and email address

C. True Copy of Grades (TCG) in all semesters enrolled and GWA certification.

Aside from at least 30 units earned, additional requirements per degree program:

• For BS Biology: GWA of 2.800 or better and no failures in the following courses: MATH 40 or MATH 53 or equivalent, Physics 81, 81.1, 82, and 82.1 or equivalent.

• For BS Statistics: Grade average of 2.000 or better in her/his Mathematics and/or Statistics courses.

Note for S1 & S2: All four documents should be submitted in a single PDF file with the file name: Surname_Shiftee_DegreeProgramAppliedFor.pdf (e.g. DelaCruz_Shiftee_BSBIO.pdf).

Note for T1: All four documents should be submitted BOTH in printed and digital copies.

Printed copies:

• • Should be sent to CS-OCS mailing address through courier from Monday to Friday (8:00 AM to 5:00 PM)
