Overview of Heartland Family Services: Professional therapy for individuals, couples, and families. Counseling Services share a central focus on therapeutic services in order to treat current mental health and substance use issues, manage chronic illnesses, and intervene earlier to limit occurrences through education and prevention services.

About: Community-based care and treatment for individuals who are challenged by chronic mental illness. Services include case management, initial and ongoing assessments, psychiatric services, employment and housing assistance, family support and education, substance abuse services, and more.

Services are provided in Pottawattamie County in Iowa.

About: The Heartland Family Service Assessment Center offers clinical evaluations for youth and adults who are involved with the legal system. Services include co-occurring substance use and mental health assessments and psychological evaluations in order to provide clients, judges, caseworkers, and other referring agencies with information and recommendations that may assist with treatment and case disposition planning.

Services are provided primarily in Douglas and Sarpy counties in Nebraska.


Eligibility: Children aged birth to 18 and their families living in Council Bluffs and the metro area.

About: Enhanced therapeutic counseling and treatment for children ages birth to 18 and their families. The goal is to help children and their families address past and ongoing trauma and improve relationships.

Services are provided in Council Bluffs, Iowa, but are open to children and families throughout the metro area.


Chemical Dependency- Family Works (HFS)

About: Long-term (6-9 months) residential substance abuse and mental health programs in Iowa and Nebraska for women with children. The program preserves and strengthens the mother-child attachment while providing housing, treatment, therapy, and support for the women and their children.

Services are offered to residents of Council Bluffs and southwest counties in Iowa and Cass, Dodge, Douglas, Sarpy, and Washington counties in Nebraska.


About: Dads build a stronger bond with your children! Family Works Iowa offers the National Fatherhood Initiative class 24-7 Dad, which is a nationally recognized program designed for any dad who wants to improve his knowledge and skills to help him raise healthy children.

Eligibility/ Requirements: Men and women ages 18 and older who reside in Fremont, Page, Mills, Montgomer, Pottawattamie, Cass, Shelby, Harrison, and Monona counties in Iowa. Participants must be eligible for the Southwest Iowa Mental Health and Disability Services Region-funded programs.

About: The Heartland Family Service Heartland Bridges program provides short-term housing for people in transition to and from Mental Health Counseling or Substance Use Treatment, support services, and/or permanent housing. The goal of the program is to help clients secure permanent housing and connect them with community-based mental health or substance abuse services and avoid unnecessary incarceration.


Eligibility/ Requirements: Participants must have or be eligible for Iowa Medicaid, have a mental illness that impacts his/her life, and reside in Pottawattamie, Harrison, or Mills counties in Council Bluffs, Iowa.

About: Care coordination for adults with severe mental illness. Integrated Health Home (IHH) partners with participants’ existing healthcare providers in order to meet overall health needs.


Eligibility/ Requirements:

Client must have a designated mental illness which results in a functional deficit in achieving and/or maintaining

a desired living, working, or social environment. Must be at least 18 years of age. The medical treatment and symptoms of the

mental diagnosis have been verified with medical records.Client displays a willingness to participate in the IPR program.

About: The Heartland Family Service Intensive Psychiatric Rehabilitation (IPR) program is designed to restore, improve, or maximize the level of functioning, independence, and quality of life for people experiencing a disabling mental illness. The goal of IPR is to assist the client with being able to use the necessary support systems to be successful and happy.



Cost: Sliding scale based on income.

Eligibility: Professional therapy for individuals, couples, and families. Services may address concerns of conduct, identity, mood, anxiety, affect, impulse control, addictive, or other behavioral concerns; other mental or emotional conditions; and adjustments to transitions in the family life cycle.

Services are provided in Harrison, Mills and Pottawattamie counties in Iowa and Cass, Dodge, Douglas, Sarpy, and Washington counties in Nebraska.


Eligibility/ Requirements: At-risk children with behavioral or mental health challenges. Students are referred by local school districts when they are no longer able to grow in their current school environment.

About: Features individual, group, and family therapeutic services facilitated by our experienced, licensed mental health counselors. A variety of trauma-informed care* and evidence-based practices* are used, including Positive Behavioral Interventions and Support and Social and Emotional Learning, to help improve a child’s ability to learn and thrive in an educational setting. One Oak also provides individualized academic support so each child is well-prepared and on-track to meet educational standards, with the goal of returning to their referring school.

*Evidence-based practices are scientifically proven effective.

About: OnTrack of the Heartland provides an innovative team approach to provide recovery-oriented counseling and treatment. The recovery team tailors treatment plans to meet the needs of each individual. The individual collaborates with the team to select the best mix of services and supports, such as:

• Assistance and support in employment and/or education goals

• Care coordination

• Education and support for families

• Individual therapy

• Medication management using shared decision making


About: Community outreach and educational activities that aim to reduce the need for intervention and treatment. The goals of these programs are to promote youth development, reduce risk-taking behaviors, build assets and resilience, and, ultimately, prevent problem behaviors throughout the life span.

About: Outpatient active treatment for adults and adolescents and long-term aftercare support that includes family and individual therapy and gender-specific groups. The dual-diagnosis approach provides on-site mental health evaluation and treatment, psychiatric evaluation, medication management, and 24-hour emergency access.

Eligibility/ Requirements: Special Education students with major mental health diagnoses. Students must be referred by their local distric within the Green Hills Area Education Agency of southwest Iowa, which includes the counties of Adair, Adams, Audubon, Carroll, Cass, Clarke, Crawford, Decatur, Fremont, Harrison, Madison, Mills, Monona, Montgomery, Page, Pottawattamie, Ringgold, Shelby, Taylor, and Union.

About: Full-day academic and therapeutic programming for special education students with major mental health diagnoses. Family involvement is heavily emphasized, and comprehensive wrap-around services are provided to assist the student and family in beginning a healing journey.

About: The Heartland Family Service Victim of Crime Assistance (VOCA) program is designed to help victims of crime, whether the crime is documented or undocumented. The ultimate goal for the VOCA program is to assist victims in their recovery from the crime they have experienced.

Overview of LFS Adult Behavioral Health: Sometimes it is helpful to talk with a professional about your difficulties. Your counselor will help you strengthen your family, develop your relationships and prevent problems from getting out of control.

About: Veterans Services programs provide confidential, individualized counseling services for active military personnel, veterans and their loved ones regardless of ability to pay.

Overview: Mental illness affects people of every gender, race, age and socioeconomic level.

Because mental illness often goes hand in hand with homelessness, joblessness, poverty and other challenges, many of our mental health programs offer not only counseling and medical support but also other services like a safe place to live, nutritious meals, life skills classes, referrals to employment opportunities and peer support groups.

Community Counseling Center (SA)

The Community Counseling Center offers free, trauma-informed individual and group therapy to those who are often overlooked for treatment in the metro area. We serve those who uninsured, homeless or have very low incomes and are enrolled in other Salvation Army programs.

Mental Health Respite offers individuals with an immediate psychiatric crisis residential care for up to 28 days, providing the support, structure, and assistance accessing services needed for stabilization.

Community Support Mental Health (SA)

This program provides care to people who are 19 years and older, have a severe and persistent mental illness, and are in need of supportive services.

Emergency Community Support (SA)

This 90-day program serves individuals who are 19 years and older and have experienced a behavioral health crisis. The goal of treatment is to achieve stabilization, increased independence and community integration.

Intensive Community Support (SA)

This program offers assistance for six to 12 months to people age 19 and older who have severe and persistent mental illness and need daily supportive services. Treatment focuses on successfully developing skills in order to maintain adequate housing, increased mental health stability and, ultimately, independent living.

Post-Crisis Response (SA)

This program provides community support services for up to 90 days for men, women and children who have recently experienced a mental health crisis that resulted in law enforcement intervention. We assist participants with stabilization, increased independence and community integration.

Eligibility/ Requirements: Must be over the age of 19 and overcoming effects of sexual exploitation, sex trafficking or prostitution.

About: Wellspring serves adults who are currently involved in or have a history of sex trafficking, prostitution, exotic dancing or survival sex. Individuals of any gender identity or sexual orientation who wish to make changes in their lives will find counseling through this program.

Therapy (SA)

Wellspring offers individual therapy in a non-judgmental, trauma-informed manner in order to help survivors recover from their lived experiences. Therapists are trained in EMDR, which is an evidence-based treatment for survivors of trauma. Chemical Dependency Evaluations (SA)

Wellspring offers free Chemical Dependency Evaluations to participants by a therapist who is well informed in dynamics related to prostitution and sex trafficking. Wellspring staff make recommendations and work to connect participants to partnering Chemical Dependency Treatment programs. We can often continue to engage participants in individual therapy while collaborating with their Chemical Dependency Treatment team.

Wellspring offers groups as a free service to the Douglas County Department of Corrections and screens participants in these groups for Wellspring program eligibility. We work to connect these individuals to our services upon their discharge from corrections.

Services are not appropriate for patients with a full IQ below 80, are currently dependent on drugs or alcohol, or have a history of sexual perpetration.
