Forms

• Fill out and sign Lab Usage Agreement (below).

• Fill out and sign Volunteer Agreement and Release (below).

• If the volunteer is a minor (under the age of 18) the individual and the lab are required by law to complete additional paperwork.

• Send all completed paperwork to CBS HR at CBSHR@umn.edu.

Training

• Safety training is handled at the University by the Department of Environment Health and Safety (DEHS).

• Safety training is required for all employees, students, and volunteers prior to starting work in the lab. Depending on the type of work that is done in the lab, different trainings are required.

• Visit the DEHS training website to find appropriate training.

Access

• Visit Key/U-Card Access Requests for space access.
