Flexible Work Arrangements

Employees:
Review the Flexible Work Arrrangements (FWA) Policy.
If eligible, choose an FWA option.
Complete the FWA Employee Request Form and arrange a time to discuss the request with your supervisor. Please note that supervisors have the option to turn down an FWA request.
Supervisors:
Review the FWA Policy and the FWA Supervisor Checklist.
Discuss the FWA request with your employee.
Send the fully signed FWA Employee Request Form to Beth Williamson (see submission deadlines below). ALL FWA Employee Request forms must be forwarded to Beth Williamson regardless of the supervisor's final decision.
Please note: If the telecommuting option is chosen, the following forms must be completed and signed by the employee and supervisor and submitted along with the Employee Request Form:
