Horse Show

Steps to complete by May 15th:

1. Make an appointment with your vet to get your documents signed & verified (by/before May 15).

2. (Re-)Enroll in 4HOnline. Use this tip sheet to help!

3. Complete the Horse ID Worksheet, 

   • Coggins test report required from vet.

   • Complete and upload the form to 4HOnline. (PDF or image file accepted)

   • Refer to this user guide for assistance

4. If your animal is leased, you must upload a Minnesota 4-H Lease Information Form into 4-H Online by May 15, 2025 with 4-H Online ID information.
