STEPS is an innovative chronic pain self-management program delivered through a workbook and online resources and coached by community health workers. The program offers support, encouragement, and information about how to take control of your life with chronic pain.
STEPS is comprised of seven weekly sessions which you can work through a workbook and online video in your own time. By wearing your activity tracker, you can monitor your progress toward increasing your daily steps. You benefit from the support of your community health worker whom you can share progress with via one face-to-face session and 6 phone sessions. To get the most from this program, we recommend you complete one session per week for six consecutive weeks. You will get a phone call each week from your community health worker. She will help you set goals and practice the new skills that you are learning.
Mike has 22 years of combined experience as a CHW and CHW Supervisor in multi-cultural communities. Born and raised in Detroit, Mike is a graduate of Northwestern High School. He also attended Wayne County Community College, Jackson Community College, and New York University.
His involvement in the struggles of the African Community stretch back to the early 60's and today he serves as Minister of Information Million Man Alumni, Member of the African Liberation Day Committee, Member of The Detroit Council of Elders, and Secretary Board of Directors Alkebu-lan Village. He is also involved with a host of other African conscience organizations.
Leslie is a Certified Medical Assistant, Associate in Diabetes Education Level 2, Intrinsic Coach, and a Community Health Worker (CHW) and her focus is to help people achieve better physical health and to also take a holistic approach to improve their health as well. Leslie educates and encourages participants to incorporate healthy lifestyle change by way of physical activity, healthy eating, and medication adherence.
As a CHW, Leslie assists people with available resources, advocates on their behalf and connects them to organizations to meet their specific needs. She empowers people to find their own inner strength to overcome any barrier or challenges that they may encounter.
