This is a Google Form that you will fill out so an LCLC Staff member can let the student's instructor know their student came in for tutoring and what you worked on.
The form goes to LCLC staff and we will contact the instructors.
You should not contact the instructor yourself.
Take notes as you talk to each student. Ask them for their instructors name, their language level, their full name and their email. Then fill out the rest of form immediately after the session.
For the open-ended parts, you can write something like:
What you worked on: "the future tense...page 47….online work exercise 6...chapter 2 pages 2-6….workbook page 7, exercise 2….vocabulary for chapter 2....Chapter 6 exam, especially the composition…"
Outcome: “She said she would talk to you in person/over email,” “He wanted to come back with more questions,” “She seemed to understand a little better how to study for the test…”
