The student provides the preceptor with mid-term and final evaluation forms. There are two components to the evaluation tool; professionalism and patient management skills. Please be sure to complete both sections.
A passing grade for the clinical experience at the end of the semester (final evaluation) depends on a passing grade for professionalism and patient management skills per ratings specific to each course.
Professionalism is graded as ‘present’ or ‘absent.’
Clinical behaviors are graded on a 0 to 4 point scale.
0 = Unable to demonstrate behavior safely and/or accuracy, even after given guidance.
1 = Needs direct guidance in demonstrating the behavior, but considered safe and accurate.
2 = Able to demonstrate behavior safely and accurately with minimal assistance.
3 = Demonstrates behavior independently, but needs encouragement to trust own judgment.
4 = Consistently independent, uses sound judgment within scope of practice and legal guidelines.
Students are expected to perform at the 1 to 2 rating level during early clinical experiences. As students progress in the program, they are expected to perform at a 3 to 4 rating level.
A clinical faculty member will perform one or more clinical site visit(s) during the student’s clinical semester. Clinical site visits provide clinical faculty with the opportunity to evaluate the student’s clinical performance via direct observation of the student’s performance and verbal communication with the clinical preceptor. During this visit students demonstrate the attainment of (or progression toward the attainment of) advanced nursing practice competencies. Clinical preceptors are encouraged to engage in discussions with site visitors regarding students’ strengths and weakness, as noted in the clinical setting.
The student is responsible for coordinating the site visit with the clinical faculty member and the clinical preceptor.
