Purpose : To apply for renting the housing and parking space after being chosen as a candidate tenant.

Application for Postponement of the Deadline of Moving into Housing
(宿舎入居期限延期申請書)
Purpose : To apply to postpone the move-in deadline when the tenant is not able to move into the housing within ten days from the date specified by the University due to illness or an accident.
* Documents which prove the fact must be attached.
Submission Deadline : By the date of permitted use listed on the "Approval for Renting Faculty/Staff Housing and Parking Space" (宿舎及び自動車保管場所貸与承認書) form.

Notification of Vacating Housing
(宿舎明渡届)
Purpose : To notify vacation of housing.
Submission Deadline : The last day of the month prior to the month of the vacation date.

Application for Postponement of the Deadline of Vacating Housing
(宿舎明渡猶予申請書)
Purpose : To apply to postpone the vacation deadline when the tenant is unable to move out within 20 days of the vacation date. Valid reason required for approval.
Submission Deadline : Ten days before date of vacation.

Application for the Mitigation of Damages Payable in Relation to Housing
(宿舎損害賠償金軽減申請書)
Purpose : To apply to reduce the amount of restitution due for the use of the housing over the postponed vacation deadline.
Submission Deadline : One month before the postponed vacation deadline.

Notification of Change in Information Provided in the Application to Rent Housing
(貸与申請書記載事項変更届出書)
Purpose : To notify of changes in information such as: persons living in the housing, designated parking space number, car model, license plate number, car owner / user, etc., provided in the Application to Rent Faculty/Staff Housing and Parking Space (宿舎及び自動車保管場所貸与申請書) form.
Submission Deadline : Within ten days after change occurs.

Application for Alteration on Housing

Purpose : To apply for permission to make alterations to the housing unit.
Submission Deadline : Ten days before the scheduled work date.

Purpose : To request repair of the housing.
Submission Deadline : When necessary.

Notification of Long-term Absence from Housing

Purpose : In case the tenant will be away from the housing for an extended period, generally one month to a year, due to overseas business trips, visiting home country, etc.
Submission Deadline : Ten days before the period of absence begins.

Report on Damage/Defiling of Housing
(宿舎の損傷又は汚損の確認・申出書)
Purpose : To report damage to the housing if the tenant finds damage upon moving in.
Submission Deadline : Within two weeks from the move-in date.

Proof of Parking Space Certificate
(保管場所使用承諾証明書(車庫証明))
Purpose : To acquire proof of parking space due to changing address, buying a new car, etc. [Example&Notice]
