This document is a Graduation Plan (for the 2025-2026 school year) that outlines your student's four-year high school course schedule. It is reviewed annually with the school counselor and the student before class scheduling.

Student Information and Signatures: Please provide the student's name, date, graduation year, phone number, and both Parent and Student signatures.
Graduation Track and Endorsement:
The student must select a Foundation with Endorsement from the following five options: Arts/Humanities, Business & Industry, Public Service, STEM, or Multidisciplinary Studies.
There is also a field for selecting Post Secondary plans: 4 year college, 2 year college, Certification Program, or Intent to Enlist.
Distinguished Level of Achievement requirements are listed: 26 credits + Algebra II + outstanding performance in one of the following: Dual Credit (DC), Advanced Placement (AP), PSAT/SAT/ACT scores, or a Certificate.
Four-Year Course Plan (Grades 9–12):
The form lists 8 courses per grade level.
It outlines the required core subjects (English, Math, Science, Social Studies) and allows for the selection of specific courses (e.g., English I vs. Honors English I, Alg 1 vs. Honors Alg 1).
