Let's be honest, no one is perfect. Every now and then you will experience a failure of some kind, and this has never been more true for engineers. Eventually you will work on a project where something will result in failure. This can range anywhere from a physical flaw, to errors in process, or even errors in perspective or attitude. In each of these cases, the result is usually that the solution design either be restructured, or redesigned. It is important that these issues are taken into account and proper analysis is done to figure out why these failures happened in the first place. Until it is found out why there was a failure, the same mistake can be made again in the future, so it is best to be taken care of as soon as possible.
