Yes - Documentation ensures that the device is available to the student when it is needed. This supports the student if he/she moves to another school district, school building, or state where the same technology may not be available. Yes - A student’s use of AT enables the student to make progress toward IEP goals and to access the curriculum. Without the tool, the student would be unable to complete curricular requirements with the same level of success.This also applies to state assessment participation. Documenting the use of assistive technology tools for assignments, tests and district assessments ensures that the student is able to access the same supports to complete state assessments, as allowed.
In the present level, add a statement that the student uses a personally owned device (laptop, tablet, augmentative communication device) and describe how the student uses that device in the school setting.
AT documentation is required in the following areas: 1. Special Considerations: Check YES that the team has considered AT. 2. PLAAFP: State why the student requires AT/what are the features of the tools)/What is the purpose? 3. If the student requires a SSD AT-dept-provided piece of equipment and/or a specialized tool/program and/or ongoing consultation with an AT Facilitator, add Support for School Personnel - Consult with Computer Access Facilitator.
As soon as possible! When completing a Facilitator Request Form or as soon as possible after the team has considered Assistive Technology to overcome barriers to performance and initiated a trial period and/or determined AT is required to access the curriculum and/or to make progress on IEP goals.
