School Dentals & Physicals

It is recommended that children entering Kdg should submit a dental and physical form for their child. Those entering 3rd grade will need to submit another dental exam form to the Health Room as well.

If you are do not have a primary care physician or dentist, or cannot afford this care, Southern Lehigh School district offers these services. Once a year, in the Spring, the school physician and dentist come into the school to do exams FREE OF CHARGE to students in need. These exams are very basic in nature and are meant to be a screening only. No procedures are done at the school. If further medical follow-up is recommended, you will be notified by the Health Room.

If you would like the school physician or dentist to assess your child, please contact the Health Room at your child's school. All information is kept confidential.

If you have not turned in a physical or dental form for your child, the Health Room will be contacting you. Please send these forms in as soon as possible. Again, if you would like the school physician or dentist to examine your child, let the Health Room know.
