Links to the district's medical forms are listed below. You will have to copy and paste them into your browser in order to get to them. However, you can also find them on the SLSD website. The directions are listed here:
Go to the main page and click on the "District" tab on the top left side of the screen.
Scroll down to the bottom and click on "More"
Then Click on "Health Services"
Then Click on "Health Forms" on the left side of the page
