Employee Injury Reporting
If you are injured while working, here is what you need to do:
Report to the nurse to make sure that no medical care is needed.
You will then need to fill out a Workman's Compensation form/Incident Report ASAP.
This is found by going to the SLSD main page
Then click on the "District" tab (top left)
Then scroll down and click on the "Human Resources" link
Then scroll down and click on the "Worker's Compensation link (all the way at the bottom)
Then scroll all the way down to the bottom of that page and click on "SDIC Worker's Compensation Claim Package"
This is a 9 page document. Print out the entire document. If you are just reporting an injury or incident with NO medical care needed, you will need to fill out page 3 and then scan/email it to Mr. Ethan Ake-Little @ akelittlee@slsd.org. All reports need to be filed within 24 HOURS of the incident. If you will be needing medical care, YOU are responsible for calling the WC provider and getting your own claim number. This sometimes cannot happen right away due to the nature of the incident, but you will need to do this as soon as possible for claim purposes.
