Employee Injury Reporting

If you are injured while working, here is what you need to do:

1.  Report to the nurse to make sure that no medical care is needed.

2. You will then need to fill out a Workman's Compensation form/Incident Report ASAP.  YOU HAVE 24 HOURS TO FILE A REPORT AFTER THE INJURY!

   1. This is found by going to the SLSD Intranet Page under "Employee Forms on the top right corner of the page.

   2. Scroll down and click on the box for Workman's Compensation.
