Each rotation, I created a fluid student info document that listed important student info, such as IEPs, 504s, GIEPs, student behavior and other important student information. This doc would be changed as time went on and finally, it would be shared and passed along to the next exploratory teacher that would be getting my group of students next.
Each rotation, weekly tracking forms were completed and turned in to the case managers assigned to specific students. Students were rated according to their everyday behaviors and actions and written feedback was documented as well.
