To create an editable version of these observation forms, open them and navigate to File > Make a copy. Rename the new copy Observation Form #_PT name. Share the new copy with your PT via Google Docs. Your PT will complete page one of the Lesson Plan and Reflection Template before the observation and complete page two of the Lesson Plan Reflection Template after the observation, before the observation debrief.
