Expectations for maintaining and IEP folder at the building:
Working files are kept organized and accessible for use by school staff.
Documents should be in date order.
The most current IEP and related documents are hole-punched and maintained in the front section of the brown file folder.
The front pocket is used for items which cannot be hole-punched.
Past documents are kept in date order in the back of the folder - oldest year on the bottom for seven years.
Each metal tabbed section is filled and then move on to the next tab.
Back folder pocket is for documents, which relate to past IEPs.
If a student moves to another district, keep file for one school year (the student may return).
When cleaning out the Working IEP File:
KEEP:
· IEP and IEP2 Signature Pages – current and past seven years
· Progress Reports & Visual Representation of Data
· Assessments and data used to develop present levels of performance and gather baseline data
· Notice of Meeting
· Notice of Attempts to Contact
· Student Invitation [16 years and older]
· Documentation of vocational assessments
· Documents related to Behavior Plan
· Visual Schedule/Social Stories/Planning Support Matrix
· Copies of the most recent evaluation documents
· Signed Releases of Information
· Records Request from another school district if the student has moved
DO NOT KEEP / SHRED:
· Notes written by staff
· Copies of birth certificates, grades, immunizations
· Office discipline referrals
· General education curriculum materials / samples
· Other documents / notes not directly related to the IEP
