The website has been updated to display the new forms and direct links are below.

New Internal Submission Form (UT Austin)

New External Submission Form (Non-UT Austin)

The new forms now allow you to select MALDI as the ionization method and add a billing line for staff-run MALDI samples. You can learn more about the new MALDI here.

We have also clarified the new forms by changing MS Resolution (High vs. Low vs. High and Low) to Data Format (Accurate Mass Report vs. Full Scan vs. Both). See the FAQs page for an explanation of the different data formats.
